Must the information in the ‘required’ feeds be factual in order to receive a free “Mold Survival Kit”?
Yes. In order to receive a pre-paid lab credit you must be setup in the PRO-LAB® system as an existing customer. Therefore, all required information must be factual as you are providing consent for a PRO-LAB® employee to call the number you provide, verify your mailing address and verify your email address as you will receive two emails from PRO-LAB® once you send in your swab.
